Don't forget, each problem as it's story...

2- Current Condition

State the problem with facts.
Demonstrate what is the problem and why it is a problem.

3- Goal / Target Condition

What is the goal you are trying to achieve. Be SMART !
Remember, conclusion will evaluate resolution based on Goal.

4- Quick Fixes

Sometimes, we needs some quick fixes.
Make sure to document them so we know they are quick fixes and not the solution.

5- Analysis / Root Causes

Use QA tools to analyse the problem and make sure you fully understand it.
Make sure you find solutions based on root causes !

6- Countermeasures

Now it's time to get creative !
Don't forget to rank each countermeasure with quantitative and qualitative criterias.

7- Action Plan

Get the right solution(s) implemented in time.
Assign tasks to team members and track deadlines.
What, Who, When and results.

8- Conclusion

It is time to evaluate if problem is solved.
Also a good time to share lessons learned and potential for standardization.
